We began 20 years ago with a knock-out exhibition at the Art Gallery of NSW which raised over a million dollars. People from the remotest, most beautiful part of the desert, then used this money to develop their own organisation, with a culture of caring, compassion and determination. At the time, the focus was to get family members home to Kintore on dialysis, for people to be looked after on country and to maintain the traditions of looking after country and passing on cultural heritage to future generations. It took a little time…and a lot of hard work…but in 2004, Purple House started dialysis in Kintore, the first of its kind, and set a precedence for many others to follow. Today we’re proud to say that we have collaborated, supported and helped many communities to get their people home and now operate dialysis in 18 communities across NT, SA and WA. We also run two mobile dialysis trucks, provide a range of support services, run a successful social enterprise and have big plans for more!We have come a long way in 20 years!About the role We’re after a self-motivated and hardworking Aboriginal Health Practitioner to join our Primary Health Care team in Alice Springs. As an AHP, you are a key member of our Primary Health Care service, which is a small multi-disciplinary team providing culturally appropriate clinical care specifically for dialysis patients and their families. You will be central to maintaining cultural integrity and advocate strongly for our patients. You will maintain strong connections with external health services that recognise and respond to the social and cultural impacts on physical, mental, emotional and spiritual health. Some of your day to day responsibilities include: Conducting patient assessments, treatment and referrals within scope of practice and in accordance with Primary Health Care Guidelines (CARPA manual)Assist in completing annual holistic health screening and chronic disease management plans in collaboration with patient and the GPAssist in coordinating internal GP, Allied health and specialist clinics and services for Purple House serviced patients Schedule and support patients to attend health care appointments as required. Advocate for patients and protect the rights of the individual Provide Health promotion and preventative health care advice to all patients Collaborate as needed with internal social support, nutrition, aged care/NDIS, care coordination teams. Maintain patient records to a high standard, including progress notes, records of communication with or regarding patients, updating medical history and demographic information. About you You have a patient centred focus and eye on our goal of getting people home on country. You are known for your compassion and empathy. Your style is flexible, good humoured and resilient. You are a self-starter and can work autonomously, knowing when to ask for help. You can think on your feet to negotiate and solve problems. You’re eager to learn and collaborate with your team.
